Sub-Flooring

My cheap parquet floors have been popping out of place for years. I finally made the decision to stop re-gluing and just replace the floor. I ripped off all the parquet thinking that I’d just have to put some new pretty flooring material down. But the plywood underneath is sort of warped and gray looking (a leaky dishwasher was recently discovered near the most troublesome spot)

The leaky dishwasher has been fixed and I think that I should replace the sub-flooring.

What is the best way to take the sub-flooring up?

What type of material makes good sub-flooring?

Do I have to take the kitchen cabinets out and redo the floor underneath them or can I just cut around the cabinets and disguise any differences in the level with trim?
